Quality has been defined as “customer satisfaction and loyalty” (or “fitness for use”).  Quality may also be defined as “conformance to specification”.  In theory, creating the proper specifications and then manufacturing product that conforms to those specifications should lead to customer satisfaction.  Alas, life isn’t that simple.

Consider these four situations for a product:

Conforms to specifications; is competitive on fitness for use

Conforms to specifications; is not competitive on fitness for use

Does not conform to specifications; is competitive on fitness for use

Does not conform to specifications; is not competitive on fitness for use

Which two of these four situations are theoretically not plausible, but in practice do occur (and cause must confusion and concern)? Can you cite any examples for all four situations?

Module 2 Question 1:

In the Failure Mode and Effect Analysis (FMEA) one of the steps is to assign 1-10 ranking to identify the severity of the failure, probability of occurrence for the failure and ability to detect the failure.  A challenge related to this step is everyone may assign different rankings to a failure, resulting in difference in overall failure ranking.  What can we do to minimize or eliminate this challenge?
